Dutasteride for hair loss: how it works, its approval status and what the evidence shows

Dutasteride for hair loss: how it works, its approval status and what the evidence shows

Dutasteride comes up regularly when the conversation turns to strong treatments for hereditary hair loss. It belongs to the same class of active ingredients as finasteride and acts at the same point in hormone metabolism, but it is considered the more effective of the two.

The most important point up front: in Germany, dutasteride is approved only for the treatment of benign enlargement of the prostate, and it is not approved for hair loss. For hereditary hair loss it is therefore used exclusively off-label, meaning outside its official field of application. The treatment approved for hair loss in men is finasteride. What dutasteride is and how it works

Dutasteride is a 5-alpha-reductase inhibitor. To understand what that means, it helps to look at the cause of hereditary hair loss (androgenetic alopecia): the hair follicles at the hairline and crown are sensitive to the hormone dihydrotestosterone (DHT). Over the years, DHT causes the affected follicles to shrink, the hairs become thinner, shorter and eventually fall out. The enzyme 5-alpha-reductase converts testosterone into DHT. Inhibiting this enzyme lowers the DHT level and thereby removes the follicles' most important attacker. There are two types of the enzyme. Finasteride mainly inhibits type II, which is active in the hair follicle among other places. Dutasteride inhibits both types and thereby lowers the DHT level in the blood more comprehensively.1 This is exactly where its reputation as the more potent treatment comes from. Approved for the prostate, off-label for hair loss

The central point with dutasteride is its approval status. In Germany, the active ingredient is approved only for the treatment of benign enlargement of the prostate (benign prostatic hyperplasia).2 It has no official field of application for hair loss here. If dutasteride is nonetheless prescribed for hereditary hair loss, this is done off-label.

Off-label means that an approved medication is used outside its official field of application. This is an established part of medical practice, but it shifts responsibility entirely to the prescribing doctor, who weighs the benefits and risks for the individual case. With dutasteride for hair loss this is particularly relevant, because the approval studies were carried out for a different condition.

In some countries, such as South Korea and Japan, dutasteride is explicitly approved for hair loss as well. That explains why there are clinical studies on it at all, but it does not change its approval status in Germany. In any case, the active ingredient is prescription-only: you can obtain it only with a doctor's prescription, and it is prescribed only if a doctor considers the treatment medically appropriate.

Does dutasteride work better than finasteride?

At the level of the hormone, the answer is clear: dutasteride lowers the DHT level more strongly than finasteride, because it inhibits both types of the enzyme.1 Whether this also results in visibly more hair is the harder question.

Shorter comparative studies suggest that with dutasteride some men achieve somewhat more hair growth than with finasteride.3 But this statement holds up only to a limited degree. Most of the studies ran over just a few months, had small numbers of participants and were in some cases open-label. Reliable long-term and safety data specifically for use against hair loss are thinner than for finasteride, for which extensive data over years are available. A greater effect on the hormone therefore does not automatically mean a better result for every user.

Side effects, half-life and safety

Because dutasteride acts on the same hormone metabolism as finasteride, their possible side effects are similar. In some men, sexual dysfunction is possible, such as reduced libido or erectile dysfunction. Mood changes are reported less often. These symptoms usually resolve after stopping the medication, but in individual cases they can persist longer and should be assessed by a doctor early on. Two properties distinguish dutasteride from finasteride here:

  • Stronger effect on DHT: Dutasteride intervenes more comprehensively in DHT balance, so that hormonally driven side effects can be more pronounced.
  • Long time in the body: The half-life is several weeks, and the active ingredient is still detectable in the blood months after the last dose.1 If side effects occur, they can therefore last longer, even after stopping the medication.
The long time in the body has consequences beyond side effects. Because the active ingredient can disrupt the development of a male fetus, men taking dutasteride must not donate blood while taking it and for up to six months after the last dose.1 When trying to conceive, too, the long time in the body is a topic that should be discussed with a doctor beforehand.

Because dutasteride is originally used for the prostate, it also lowers the PSA level in the blood, a marker used in prostate cancer screening, by roughly half.1 Anyone undergoing such screenings should inform their doctor about taking it, so that the value is interpreted correctly.

In 2025, the authorities updated the safety information for this class of active ingredients. A Dear Doctor Letter (Rote-Hand-Brief) confirms, for orally taken finasteride, above all in its use against hair loss, depressed mood and suicidal thoughts as a side effect; the frequency of the suicidal thoughts is not yet conclusively known. For dutasteride the same warning applies as a precaution, because it belongs to the same class of active ingredients; a direct connection has so far not been sufficiently established here.4 This is no reason to panic, but it is a reason to have such treatment medically supervised and to raise any new symptoms early.

Why dutasteride is not an option for women

5-alpha-reductase inhibitors are contraindicated in women who are pregnant or could become pregnant. The reason is clear: the active ingredient can disrupt the genital development of a male fetus.2 That is why dutasteride is not an option for women of childbearing age with hereditary hair loss. With dutasteride the long time in the body adds to the concern, because the active ingredient is still present in the body months after stopping.

Hair loss in women often has different causes and is treated differently than in men. Which options are then appropriate is determined by the medical health questionnaire in the individual case.

For whom dutasteride is considered at all

Because dutasteride for hair loss is off-label and the data are limited, it is not the starting point of a treatment. It is considered above all in men for whom finasteride, the approved treatment, has been exhausted and still does not work satisfactorily. Whether switching to the more potent treatment makes sense in an individual case is decided by a doctor, not by the person affected and certainly not by an unregulated source of supply.
